چکیده

During the last three decades, video-assisted thoracoscopic surgery (VATS) has been revolutionizing the surgical treatment of several thoracic pathologies. Compared to open thoracotomy, VATS has demonstrated not only a significant reduction in pain, recovery time and complications but also a significant improvement in the post-operative quality of life for patients. The uniportal VATS technique was initially described in the early 2000s. This technique involves the simultaneous introduction of instruments parallel to a thoracoscope through one small incision, the breadth of a surgeon's finger, without further dissection of the intercostal space. Some papers have already demonstrated the advantage of uniportal VATS in comparison to the traditional three-port techniques in reducing postoperative pain, length of hospital stay and time of return to activities of daily living. Standard three-port VATS has a geometric configuration of a trapezoid that interferes with the optical source by creating a new optical plane which generates a torsion angle not favorable with standard two-dimensional monitors. By contrast, the uniportal VATS approach along a sagittal plane from a caudo-cranial perspective enables a projective plane that preserves the depth of intraoperative visualization. The instruments, as parallel lines from this plane, enable the surgeon to bring the operative fulcrum inside the chest. In addition, the uniportal VATS approach can significantly improve the surgeons' body posture during surgery since the surgeons can stand straight facing the monitor with minimal neck movement. Thus, the surgeons benefit from ergonomic advantages compared to the standard three-port approach.

Uniportal video-assisted thoracoscopic sleeve resection.

With continually growing interest in video-assisted thoracoscopic surgery (VATS) sleeve procedures in the international community, mastery of uniportal VATS sleeve resections is still a complex issue for surgeons. Several articles published recently have shown that VATS sleeve procedures can be performed safely (1-3). In this article the technique of a complex uniportal VATS sleeve resection is...

Modified uniportal video-assisted thoracoscopic surgery (VATS).

Video-assisted thoracoscopic surgery (VATS) for resectable lung cancer patients has been frequently used in the past decades. The potential beneficial advantages and safety of VATS has been shown in large patient series and meta-analyses. The strategy of limiting access to one incision in one intercostal space (uniportal VATS) has been adopted by some thoracic surgeons in recent years. We have ...

Comparison of Video-Assisted Thoracoscopic Surgery and Intrapleural Urokinase as an Initial Treatment for Parapneumonic Effusion and Thoracic Empyema

Introduction: The treatment of complicated parapneumonic effusion (PPE) and thoracic empyema (TE) is controversial; and the choice of treatment after confirming the failure of simple drainage remains unclear. The purpose of this study was to compare the outcomes of intrapleural urokinase (UK) administration and video-assisted thoracoscopic surgery (VATS) as initial treatment options for PPE and...

Uniportal video-assisted thoracoscopic surgery: safety, efficacy and learning curve during the first 250 cases in Quebec, Canada.

BACKGROUND Video-assisted thoracoscopic surgery (VATS) using a single incision (uniportal) may result in better pain control, earlier mobilization and shorter hospital stays. Here, we review the safety and efficiency of our initial experience with uniportal VATS and evaluate our learning curve. METHODS We conducted a retrospective review of uniportal VATS using a prospectively maintained depa...

Systematic review and meta-analysis of uniportal versus multiportal video-assisted thoracoscopic lobectomy for lung cancer.

BACKGROUND Uniportal video-assisted thoracoscopic surgery (VATS) has emerged as a less invasive alternative to the conventional multiportal approach in the treatment of lung cancer. The benefits of this uniportal technique have not yet been characterized in patients undergoing VATS lobectomy. This meta-analysis aimed to compare the clinical outcomes of uniportal and multiportal VATS lobectomy f...
